I think the web expert you should consult is Karen McCall, http://www.iprimus.ca/~martha/PDF-resources.htm  She is a regular presenter at the Accessing Higher Ground Conference in Boulder, CO. I attended her session in November and found her to have the BEST information on this topic.
 
We have also been exploring a new program know as Common Look, http://www.eliquo.ca/access/acrobataccess.htm which I've also found to be an impressive tool.
