I would look at the Quality Indicators of Assistive Technology Model (QIAT<http://qiat.org/>) that uses the Student, Environment, Task and Tools Framework SETT<http://www.techpotential.net/assessment>. Also Georgia’s Tools for Life<http://www.gatfl.org/> project has a good tool as does the Wisconsin<http://www.wati.org/> AT Initiative.  It depends on what you are needing, I have used a combination of the QIAT/SETT and the other two.

Does anyone out there have an assessment tool for AT that you would be willing to share, or links to resources that may be helpful with the creation of such a tool? In the short term what I need is an actual tool for assessing the effectiveness/usefulness of AT that is being used; but, for the long term, tools such as intake and programmatic assessments would be helpful as well.
