Just adding my two cents -- we’ve been using Rev for a few months now, and I’ve had very few issues with accuracy. You also get to rate the transcriber after each job, so I at least feel like I’m able to give feedback on the quality easily (although to be fair I’m not sure how that rating is acted upon internally). My informal assessment is that accuracy is at least on par with the other vendors if not slightly better. They also have (in my view) the most usable caption editor of any of the vendors we’ve used, plus it offers some neat features like vertical caption placement that you can’t get from the others without an upcharge. There are still specific use cases under which I’ll use each of our 3 caption vendors, but given the price and turnaround time, Rev is currently my go-to for a standard captioning request.

What our procedures are, etc. The way he was asking, I assumed he was a student or a parent of a student. Another person on a different list serv that I belong to said the transcripts they received were full of errors.

My take from reading the online reviews: The low rate they charge to customers is directly related to the low wages they pay their employees, which is the reason for the errors in the transcripts. This is all I know about them.
