Like Sean Loraas, I love the Central Access Reader. It has many robust features. The only downside right now is that it hasn't been updated yet for the latest Mac versions.

Russell,
Wanted to mention my go to mp3 recorder: Central Access Reader (CAR) is
excellent, you can also break up documents and save each page to a separate
MP3 file. Balaboka has more powerful controls for splitting large texts.
But CAR will read math imported from MS Word, with either equation editor
or MathType (or both). It also HTML with MathML that is remarkably
accessible with default screen reader/browsers on most devices and opp
systems. Hope that helps.
