is the same form that is available and fillable online, I don't believe you have
to worry about making the PDF version accessible in addition, since as you said
the intent is for the PDF version to be printed and then filled out, and an
online / accessible / html version of the same form is already provided for all
users, including users with disabilities. In other words, you have already
provided equal and equivalent access to all. Pls make sure that the online
version is indeed the same as the PDF and is fully accessible and one who is
visually impaired / screen reader user can use it
