I didn’t notice any difference between the two tools and have used the Office add-in exclusively for the past two years. It is just so easy to F12 and choose PDF as the file type. The only thing I noticed is that the graphics sometimes get put at the top of the reading order so they are read first (with alt text) before the text. I've heard from some folks that the Word Add-in works fine and just as well as the PDFMaker Add-in.
