[Athen] Alt Format Survey

This effort it US based, not sure if it is going to be replicated in the UK.
I would think that ATHEN would want their own data, I am not going to modify
the AHEAD survey at all due to this.  I am not sure how the final data will
be disiminated at this point, we will have to see what the n is first.
