[Athen] AT Intake Form

I would be interested in know if anyone has a form or process they follow
when doing an intake with students needing AT. Our current procedure is
somewhat random and could use some assistance. One of the needs we find in
determining what AT the student needs without missing some of the key issues
and needs to accommodate their whole need and not just the first couple of
things that come up. Also to make sure we are addressing the most important
accommodations and not just the one they think they need.
