As long as the content is programmatically defined in Word or Acrobat as Spanish, and the user has a Spanish language speech output installed in JAWS or NVDA, the text will be spoken with the appropriate output language.

Hello,

If you chop and scan the text book, then perform optical care recognition(OCR) in Adobe Acrobat Professional you can set the Language setting to Spanish in this document. Jaws will need to be set to read Spanish also, or using NVDA set to Spanish.
