Here's Formstack's VPAT<https://www.formstack.com/docs/FormstackVPAT.pdf> and it looks great from this report completed by a 3rd party.  We are using it quite a bit on our campus and so far, so good but we haven't done "human testing" yet.

I once used Doodle to create a pole and that worked well. However, I only used very simple elements, so that may not be the best evaluation.

Could anyone recommend/suggest form builders & fillers, which meets compliance?

We are familiar with Survey Monkey, which I believe is accessible (unless I am wrong), but are there any other alternatives?

We also need to make sure our data collections are secure, of course.
