[Athen] Software testing reporting

Our DSS/AT (Disability Support Services/Assistive Technology) department has gone through an office merge and a new director hired. These changes now include our reporting documents. Columbia Basin College tests all software for purchase or renewal. We created a template report that we send out after we are done testing. We are starting to show on our reports that if is accessible, it will be noted as  "valid for one year only" so we don't let software slip through the cracks if it is a one-time purchase versus a yearly renewal.

What I would like to know from those campuses who do software testing, what does your report look like?  How do you report on your findings of your software testing?

If anyone is really interested, I will share our template report with you on a one-on-one basis.  We are always looking to improve our reporting so appreciate any help anyone wants to share with me.
