Dear, Colleagues:

I have a few questions regarding student files and documentation:


*         How does your college maintain student files for disability/accommodation services?  Electronic, paper or both?  Paper (but we have a system of notes that are electronic)

*         If files are stored electronically - where are they stored (secure drive, etc.)?  On the college's server (only notes, everything else is paper)

*         If students request a copy of their file (or documentation) is that provided to them?  If so, is there a fee and what is the cost?  We will give them copies free of their documentation ONLY if they hand delivered the documentation.  If the documentation came directly from an agency/school, we do not release it back to the student.
