[Athen] equipment loan policies?

We have equipment for students to borrow while on campus. Such as FM Systems, bar magnifiers, CCTV, recorders etc.
We do not loan out laptops or any software. If a student needs to use software such as JAWS, ZoomText etc. in the classroom we will have it installed on a computer in the room/s for them to use.
Our policy states that students can borrow the equipment from the assistive technology lab but they are required to return it before leaving the campus. It is not to be taken with them. I will even move the item to at any of our other campuses to make it easier on the student to borrow.
The only exception we have made to this is the use of a Livescribe pen. We will loan out the pen for the semester.
We have a loaner agreement for students to sign and in the case of the pen I created a separate form that states they will be responsible for the replacement cost of the pen if it lost or stolen.
It is difficult to replace damaged or not returned equipment especially being a 2 year community college with limited funds in the assistive technology department.
For those who may not be able to afford their own personal assistive technology I inform them about a program in Massachusetts called MassMatch which may help the person in obtaining the necessary assistive technology. There is also an Easter Seal's assistive technology loan program that may be able to help them out with low interest cask loans.
I hope this is of some help

Subject: [Athen] equipment loan policies?
Hello Fellow Athenites!
I'm wondering if any of you/your colleges have an equipment loan program (CCTVs? (portable or ?), laptops with AT installed, ALDs, etc. And, if you do, do you have a written policy/statement of rights and responsibilities for students, and do you have them sign something before they are loaned the equipment? I hope this makes sense. And, would you be willing to share those documents with me so I have some idea of how to craft our documents.
We're finding more and more of our students in disadvantaged situations, and where they do not qualify for state services for a variety of reasons. In the case of low vision who could utilize a portable CCTV such as a Topaz PHD or like, it would seem advantageous to allow them to check out a portable device to carry between campuses, classes and home. Yes, I know... 3,000$ +/- is a lot for replacing a piece of equipment that may or may not come back in a workable state but...
Thank you in advance.
